Age 17 and under: $40/mo (with a parent in the practice).

Adults age 18-64: $88/month

Adults age 65 and up: $110/month

One-time $100 registration fee per household.

Pay however you like: monthly, semi-annually or yearly. 

That price includes full primary care: your annual physical, all office visits, and after hours phone access for urgent matters.

Big family? After the 3rd kiddo, the 4th or more is included (contact us for the discount). 

Think you can afford more?  Some choose to pay a little more per month. Just a few dollars goes a long way towards helping us care for the underserved. Contact us for details.
